The mission of Mobile Healthcare Partners is to offer quality, convenient, detailed, and service-oriented healthcare solutions to Western New York and the surrounding areas.

By utilizing state-of-the-art medical technologies, we are improving and revolutionizing the healthcare delivery system across both local and nationwide communities. We are committed to providing quality, flexible medical care in a supportive, team-oriented environment.

OUR STORY
Who We Are

Founded in 2007 as Mobile Primary Care by Dr. Cory Shaffer, Michelle Brenner, and Susan Oesterle, the organization aimed to deliver top-notch primary care to patients confined to their homes. The practice rapidly expanded and addressed a critical need for elderly and chronically ill patients in Western New York.

Our model employs a team-based approach, involving physicians, physician assistants, nurse practitioners and pharmacists who collaborate closely with patients and their families to ensure the most effective, comprehensive, and holistic care.

In 2025, nearly 18 years after our founding, Mobile Primary Care has transformed into Mobile HealthCare Partners Inc., a change that signifies our expanded range of services and a heightened commitment to delivering the most adaptable and convenient care for everyone requiring medical attention within the Greater Buffalo/Niagara Region.
 
We boasts over 75 employees, including numerous providers and patient care coordinators.  Beyond delivering exceptional healthcare, Mobile HealthCare Partners and our associated services have significantly reduced unnecessary emergency and hospital visits, thereby lowering healthcare costs and allowing patients to remain comfortably at home.
 
Our impact is the driving force behind our mission. We understand the growing demand for this type of healthcare and strive to offer the best possible care for our patients and their families.  By collaborating closely with patients, families, other
healthcare providers, and insurance companies, we can profoundly influence our patients' lives.  Our efforts enable patients to maintain their independence longer and contribute to reducing the escalating costs of healthcare in America.
 
In recent years, our company has grown and evolved to offer a variety of services, ranging from our core service of home visits to our upcoming Primary Care Center. Our commitment to providing flexible, convenient, and accessible healthcare for everyone has never been stronger. Our Proactive Care Management and TeleMed Virtual Care services extend our reach to assist those with chronic health issues and those facing challenges due to rural locations or transportation limitations.
 
Starting in 2024, we have broadened our services to include Mobile WellLife Medicine. This holistic approach provides patients with pathways to success in managing weight loss and ADHD, offering support from adulthood into their later years.

Currently, we cover over 60 zip codes across three counties and more than 30 cities and townships in the greater Buffalo/Niagara region. Our dedicated team of healthcare professionals consists of 30 providers who care for 2,000 patients in their homes, assisted living facilities, or skilled nursing facilities.
